When applicable: Every member must nominate within three months of becoming a member.
Completion instructions: Obtain the nomination in the manner specified by the Articles and retain the signed document. If a nominee is not a producer, the statutory surrender provisions require attention on death.
Editable preparation format. It is not a verbatim reproduction of a prescribed MCA form or a filing service. Where a statutory form is prescribed, reconcile all fields and use its current prescribed layout for the formal register. Maintain required authentication, access controls and retention.
